The patient was working
as a professional trainer.

Despite engaging in
high-intensity strength training
due to her occupation,
she remained concerned about
abdominal fat
and her pelvic contour.

Even with weight reduction
and muscle strengthening,
there were clear limitations.
In particular, the influence of
visceral fat
caused mild forward protrusion
of the abdominal wall,
indicating concerns beyond
subcutaneous fat alone.

In the pelvic area,
insufficient volume
made the saddlebags region
appear as the starting point
of the legs,
creating the impression
of shorter leg length.

From the front view,
the line connecting
the waist, pelvis, and thighs
appeared blunt
and slightly uneven.

Given her profession as a trainer,
where body balance
and contour refinement are essential,
the surgical plan focused on
reducing abdominal volume
to create a slimmer waistline,
while adding volume
from the flanks to the pelvis
to achieve a slender yet
healthy and athletic silhouette.

1. Lower abdominal protrusion

The lower abdomen was visibly bulging
and felt firm and dense.
Due to the influence of visceral fat,
the abdominal wall was pushed forward,
preventing the abdomen
from appearing flat.
Postoperative dietary management
was therefore emphasized as essential.

2. Flank contour
Although muscle definition was clear,
fat along the flanks prevented
the waistline from tapering inward,
resulting in a less defined curve.

3. Insufficient pelvic volume
Because of the hip dip deformity,
pelvic volume appeared lacking
from the front view.
The line from the waist to the pelvis
looked uneven,
and the legs appeared shorter.

Based on these findings,
the plan was to reduce
waist circumference through
abdominal liposuction,
while performing
hip dip fat transfer
to restore natural pelvic volume
and improve overall proportion.

Before surgery,
the patient’s waistline was not clearly defined,
and the hip dip deformity
created a blunt, unbalanced silhouette
that required proportion correction.

After abdominal liposuction
and hip dip fat transfer,
the waist circumference was reduced,
forming a slimmer, more contoured shape.
Volume at the lateral hip
was naturally restored,
creating a smooth
waist-to-pelvis curve.

With abdominal fat reduction,
the heavy and dull upper body impression
was eliminated,
resulting in a firmer, more feminine
and well-trained waistline appearance.

Most notably,
the added volume through
hip dip fat transfer
created a more three-dimensional
and stable pelvic proportion.

From the back view,
the legs appear noticeably longer,
and the firm hips and pelvic contour
convey a healthy, athletic silhouette.

Although she is a trainer,
the natural connection between
the waist, pelvis, and hips
which is difficult to achieve
through exercise alone—
has now been completed.

With added pelvic volume
above the former saddlebag area,
the starting point of the legs
appears elevated,
making the legs look longer
and creating a lifted hip shape.

This proportional change
was one of the most satisfying results
for the patient.

In particular, from the side view,
you can clearly see
the volumetric curve change
created by hip dip fat transfer.

Beyond the frontal pelvic contour effect,
the side profile now shows
a smooth, natural transition
between the hips and pelvis,
without any visible depressions.

Even though volume was added,
the overall proportion appears
slimmer and more balanced.

The above before-and-after photos
were taken at 2 weeks post-op,
a period when swelling
and BioBond firmness
are still significant.

Despite this early stage,
the overall line change
is already noticeable,
with improved proportion and stability,
which the patient found
highly satisfying.

However, the patient’s
abdominal protrusion
was not caused solely
by subcutaneous fat
addressed through liposuction,
but also by visceral fat–related
abdominal wall projection
.

For this reason,
ongoing dietary management
remains an essential part
of maintaining long-term results.
