This patient was a university student preparing to become a flight attendant.
She hoped to achieve a silhouette with slim thighs
while maintaining volume in the hips.
She had received fat-dissolving injections 3 to 5 times about
a year prior to her surgery date but reported no noticeable results.
She later started working out, but felt it was difficult to reach the body line
she desired, so she ultimately decided on liposuction.
She visited our clinic based on a friend’s recommendation.

At the time of consultation, the patient was 172 cm tall and weighed 65.9 kg.
Her weight had previously ranged from a maximum of 70 kg to a minimum of 52 kg.
She was most concerned about the fat in her inner thighs and the inside of her knees.
She wanted her thighs to appear slimmer through liposuction
but also hoped to retain some volume in the outer thigh area to maintain a smooth
connection to the hips.
Due to prominent hip dips, we recommended hip fat grafting,
as liposuction alone would likely not achieve her desired contour.
Her buttocks also had uneven volume due to excess fat,
so we suggested liposuction for the buttocks as well.

Since she had thin skin, we anticipated that bruising
would be more pronounced following the procedure.
The patient was positive about the proposed fat grafting and liposuction plan and
decided to proceed with surgery.
The surgery was designed to remove approximately 5,400cc of fat from the thighs and
buttocks, and to graft 170cc and 140cc of fat into the hip dips.

In the patient’s preoperative photos, we can observe
that excess fat in the thighs created a rounded, barrel-like shape.
From the back, uneven contours of the buttocks were noticeable.
By removing excess fat from the thighs and buttocks and grafting fat into the hip dips,
we expected to achieve the patient’s desired silhouette.
On day 14 post-op, while bruising was visibly prominent in the treated areas,
we could already see a more refined shape in the thighs and buttocks.
By around 5 months post-surgery, swelling and bruising had greatly improved,
and the hips appeared fuller while the thighs had transformed into a much slimmer contour

What is Hip Fat Grafting?
Hip fat grafting is a procedure that fills the hollow area between the pelvic bone and the femur.
It is also referred to as pelvic fat grafting, hip dip correction, or lateral hip augmentation.

This procedure is suitable for individuals who are dissatisfied
with the line between their hips and thighs, experience indentations
due to their bone structure, or have uneven fat distribution around the hips.
Some patients choose to undergo hip fat grafting alone,
while others combine it with liposuction of the thighs or abdomen.
When performed together, the fat removed during liposuction can be reused for grafting
without the need for separate harvesting.

Combining fat grafting and liposuction often creates a synergistic effect,

In this case, the patient was able to achieve a more
satisfying result thanks to the combination of thigh liposuction and hip fat grafting.

By looking at the patient’s pre-surgery, treatment day,
and follow-up photos, we can clearly see the results of hip fat grafting.
Before the procedure, the hip dips created an uneven transition from th
pelvis to the thighs when viewed from both the front and back.
By grafting fat harvested from the thighs into the hip dips,
we enhanced the volume of the hips and created a more balanced line.
We also smoothed out the “saddlebags” on the outer thighs,
and the hip grafting visually raised the starting point of the legs—giving
the appearance of longer legs overall.

Hip fat grafting is customized for each patient,
taking into consideration skin elasticity, muscle shape, fat volume,
pelvic bone size, and the patient’s aesthetic goals.
Because the pelvic bone and muscles are not externally visible,
the surgeon’s experience and anatomical understanding are critical for success.
