What is gynecomastia?

Gynecomastia is a condition
in which men develop a female-like chest contour,
resulting in noticeable changes in body shape.
It is most commonly caused by
hormonal imbalance.

When chest enlargement is due to
excess body fat,
exercise and dieting may be effective.
However, true gynecomastia caused by glandular growth
requires surgical treatment as a definitive solution.
For patients seeking treatment,
a proper medical diagnosis
and a carefully planned surgical approach
are essential.

The patient introduced in today’s case
had persistent chest protrusion
that did not improve despite
consistent weight control
and regular chest muscle training.

Rather than withdrawing,
seeking early diagnosis and consultation
can be a positive and empowering step.
While the final decision for surgery
always belongs to the patient,
knowing that a solution exists
can itself provide meaningful reassurance.

Furthermore, by improving gynecomastia
through chest size reduction,
and achieving chest contour refinement
commonly referred to as male chest contouring,
this approach may also contribute
to the confidence boost mentioned earlier.

Despite long-term efforts,
there was little to no improvement
in gynecomastia-related symptoms.
As psychological stress and discomfort increased,
the patient ultimately decided
to proceed with surgery
and visited Lesarts Plastic Surgery.

The patient had a muscular build overall
and maintained a firm, lean body
through consistent exercise and training.

However, since his teenage years,
he experienced chest enlargement
and nipple protrusion.
Despite ongoing exercise and body management,
these chest-related concerns
did not improve.

Although the rest of his body
was well trained,
the chest area alone
protruded in a way
that felt uncomfortable and embarrassing.
He wished to fully correct
the expanded areola
and bulging chest contour.

On physical examination,
a firm mass was palpated
beneath the chest area.
Ultrasound examination confirmed
that this tissue was overgrown glandular tissue.

Based on these findings,
our medical team diagnosed the patient with
true gynecomastia, Simon grade 2A.
The surgical plan focused on
removal of the developed glandular tissue,
combined with liposuction of the lower and lateral chest,
to reduce chest size
and reshape the upper body into a more masculine contour.

[Surgical approach for true gynecomastia correction, Lesarts Plastic Surgery]

1. Gland excision
The procedure is performed through a periareolar incision.
By removing the glandular tissue, the primary cause
of chest enlargement, overall chest size is reduced.
At the same time, the bulging nipple–areola complex
can be refined for a more balanced appearance.

2. Gynecomastia liposuction
Fat distributed along the lower and lateral chest
is removed to create a smooth and natural connection
across the upper body contour.

3. Tissue repositioning (male chest contouring)
After gland removal, measures are taken to prevent
irregular adhesions or depressions around the nipple–areola area.
With adhesion prevention and tissue repositioning,
the upper chest line is shaped to resemble
a naturally trained, athletic contour.

At 2 months after gynecomastia surgery,
the patient’s chest size reduction
appears natural and well balanced,
while the overall upper body contour
has become noticeably cleaner and firmer.

Before surgery, despite consistent exercise
and a naturally muscular physique,
gynecomastia made it difficult
for his strengths to stand out.

With correction of chest protrusion
and the added effect of male chest contouring,
the upper body now presents
a more confident and athletic appearance.

Protrusion of the nipple and areola
has also been reduced,
which is expected to relieve discomfort
and self-consciousness when wearing clothing.

When comparing the
before and after appearance in a T-shirt,
the preoperative chest showed
protruding nipples and areola,
creating a pointed contour
that felt visibly uncomfortable.

With gynecomastia surgery
and the added male chest contouring effect,
the chest line became flatter yet natural,
resulting in a cleaner,
more masculine fit.

Despite a well-trained physique,
gynecomastia had previously disrupted
upper body balance.
After correction, this imbalance was resolved,
leading to high patient satisfaction.

Postoperative care and considerations
The patient has an athletic build
with high muscle mass
and regularly consumed protein supplements.
While not a direct cause,
certain ingredients were reviewed
and precautions were re-explained,
along with the importance of follow-up.
