It is true that men with gynecomastia often try to reduce their chest size by focusing on diet control and chest exercises as soon as they notice the protrusion of their chest.

However, even with dieting, if the chest size does not improve, many end up seeking surgery consultations to address their gynecomastia symptoms.

When diet and exercise are combined, it is possible to reduce the chest size in cases of pseudogynecomastia, where fat accumulates in the chest area without any enlargement of the glandular tissue. In such cases, diet alone can improve the chest size.

However, true gynecomastia is not simply an issue of fat accumulation. It is caused by the development of glandular tissue due to factors such as hormones, endocrine factors, or medication side effects. In such cases, the problem can only be improved through gynecomastia surgery, not through diet and exercise alone.

Today, let’s look at the gynecomastia surgery case of a patient diagnosed with Simon Grade 2A.

This patient is in his mid-20s and has been experiencing gynecomastia symptoms since his teenage years due to the protrusion of both sides of his chest.

He reported feeling continuous discomfort as an adult because of the protruding chest. Despite consistent efforts through bodybuilding, various exercises, and dieting to control his chest size, he did not notice any significant changes in the size or shape of his chest.

When he visited our clinic, he had experienced slight weight gain, and lumps had begun to form around his areola, which further aggravated his discomfort.

As a result, he decided to seek surgical treatment to address his gynecomastia and came to our clinic for consultation.

During the consultation for gynecomastia surgery, it was visually confirmed that the degree of chest protrusion was quite noticeable.

The diagnosis revealed that the patient was at stage 2A of Simon’s scale. An ultrasound examination also indicated that both sides of the chest required glandular tissue removal, confirming true gynecomastia.

Additionally, the patient was diagnosed with a complex form of gynecomastia due to the significant accumulation of fat in the lower chest and sides.

Therefore, we explained that the patient would undergo glandular tissue excision to address the underlying volume issue and that tissue reallocation surgery would be performed to prevent any chest shape collapse.

Below are the before-and-after photos taken 6 months after the gynecomastia surgery.

When comparing the before and after 6 months photos of the patient’s gynecomastia surgery, it can be seen that, prior to the surgery, the chest was protruding due to glandular hypertrophy, but after the surgery, it has been corrected to a flatter appearance.

Especially, the patient, who was once afraid of wearing only a thin T-shirt before the surgery, is now expected to confidently wear it without feeling self-conscious, as the volume of the chest has been eliminated and it has flattened out after the gynecomastia surgery.

Over the 6 months following the surgery, the patient has consistently attended follow-up appointments, confirming that the surgical area is healing well and steadily.

For further improvement in chest contour, I also emphasized the importance of regular chest exercises and a balanced diet.

While diet and exercise can help reduce chest fat and prevent sagging, the main cause of gynecomastia, the glandular tissue, must be removed through surgery.

Therefore, if you suspect symptoms like chest protrusion or lumps, it is essential to first consult with a medical professional to address the issue.
