This patient, in her mid-40s, had given birth twice
by cesarean section many years ago and had long suffered
from lower-abdominal sagging and C-section scarring.

Although her weight and body shape had not changed much
since childbirth, she continued to struggle with
lower belly skin laxity, a prominent C-section scar,
and an abdomen lacking firmness,
which remained major concerns for her.

To improve the sagging and bulging shape
of her lower abdomen, she had previously received
fat-dissolving injection treatments.

However, she did not achieve the improvement she hoped for,
and she realized that simply reducing fat
would not fundamentally resolve the issue.

Upon examination, her abdominal condition required
correction of skin laxity and abdominal wall relaxation.
For this reason, a surgical plan was established to perform
abdominoplasty, rectus muscle repair,
and abdominal liposuction together.

When examining the patient’s abdomen
before surgery, the area below the navel
showed a clear bulging shape
where fat and loose skin had gathered.

In a forward-bending posture,
the abdominal wall pushed outward
while the sagging skin folded without support,
creating deep horizontal creases
across the abdomen.

During this process, the high-positioned C-section scar
became more visible, making the entire
abdominal line uneven and irregular.

In other words, it was not simply fat accumulation—
skin laxity, rectus diastasis,
and the location of the scar
were all contributing to an unstable
and distorted abdominal contour.

To improve this abdominal condition:

1. S-type abdominoplasty + umbilical floating + rectus muscle repair

The S-type tummy tuck was planned
to lift the sagging lower abdominal skin,
creating a firm abdominal shape.
At the same time, the high C-section scar
was removed and the incision line
was lowered so it would be concealed
under clothing.

Since the patient’s original navel position
was not low, the floating technique
was applied to tighten the rectus muscles
that had separated above the navel
,
preventing the abdominal wall
from bulging forward.


2. Full abdominal liposuction

Considering skin thickness and fat distribution,
liposuction of the entire abdomen was performed
to create a clearer and more defined waistline.


3. Fat grafting above the navel

After abdominoplasty and liposuction,
the lower abdomen becomes smooth,
but the area above the navel
can appear sunken as fat decreases
and the muscle layer moves closer
following surgery.

Since the area originally contained
very little fat, it was expected
to appear even more hollow afterward.

To correct this naturally,
a small amount of fat harvested
during abdominal liposuction
was grafted above the navel
to restore gentle volume
and allow the abdominal line
to connect smoothly.

The previously bulging lower abdomen
was reshaped through abdominoplasty
and liposuction, and we will now examine
the changes through the 3-month
before-and-after comparison
.

In the pre-surgery photos,
lower abdominal sagging is visible along with an
irregularly shaped and high-positioned C-section scar.

Through the performed abdominoplasty
and abdominal liposuction,
the sagging and bulging that had been concentrated
in the lower abdomen were corrected,
and the C-section scar was removed as well.

In particular, the now clearer waistline
creates an effect that makes the legs
appear longer.

When bending forward, the previously visible
abdominal wall bulging and skin sagging
have now been firmly corrected,
and the layered upper-abdominal creases
have completely disappeared.

As the bulging that had shifted downward
into the lower abdomen was resolved,
the excessive folding of the skin
also corrected itself naturally,
resulting in a smooth abdomen
with a clean, continuous line.

From the side view as well,
the improvement in the lower-abdominal bulging
and uneven contour is clearly visible,
showing a much smoother line.
