Men who visit the clinic for enlarged chest
or nipple protrusion often expect
typical, symmetrical gynecomastia,
where both sides of the chest
are similarly developed.

However, many patients actually suffer from
asymmetrical gynecomastia,
where one side is more enlarged or sagging,
creating a visible imbalance.

When wearing clothes, the difference
can appear more pronounced on one side,
with the chest line visibly uneven.
This often causes stress, but it is important
to know that with accurate diagnosis
and proper surgical correction,
these issues can be effectively improved.

In this post, we will review the case
of a patient with asymmetrical gynecomastia,
detailing the process from diagnosis
to surgery and post-op transformation.

The patient, a man in his late 30s,
had noticed chest protrusion
on both sides since adolescence.

Although he consistently maintained
exercise and diet control,
there was no visible improvement
in his chest contour.
Over time, the size difference
between the two sides became
more pronounced, and this asymmetry
started to bother him more.

In addition, the nipple area
appeared protruded, and the chest bulge
was visible even through clothing.
As a result, he experienced
growing discomfort and stress
in daily life.

Realizing that these concerns
could not be resolved through self-effort alone,
he decided to seek accurate diagnosis
and confirm the proper treatment plan.

During consultation, both sides of the
patient’s chest showed visible protrusion,
with a clear size difference between
left and right.

The nipple and areola area were also
swollen, with the left side appearing
larger and more prominent.

On palpation, firm glandular tissue was
detected beneath the nipple, which had
caused long-term discomfort.

Ultrasound confirmed glandular overgrowth
on both sides, with the left side
extending wider and deeper.

The right side was diagnosed as
Simon Grade 2A gynecomastia, while
the left side was Simon Grade 2B.

We recommended the following surgical plan:

1.Gland Excision – through an areolar
incision, the enlarged gland tissue
was removed to reduce protrusion.

2.Chest Liposuction – fat in the
lower chest and lateral chest was
removed to achieve a flat contour.

3.Adhesion & Depression Prevention
tissue repositioning was performed to
avoid hollowness and ensure smooth results.

4.Finasteride & Recurrence Risk – since
the patient takes finasteride for hair loss,
we explained the possibility of recurrence
and the need for regular follow-up.

At 2 months post-op, photos show clear
improvement in chest contour and
overall symptom relief.

Before male chest reduction surgery,
the chest showed clear protrusion,
with the left side more prominent,
extending below the nipple area.

At 2 months post-op, the protrusion
was significantly reduced, and the
volume around the nipple appeared
natural and refined.

The asymmetry between left and right
was also visibly corrected. The area
where gland tissue was removed
remained smooth without hollowness,
resulting in a stable, firm chest line.

Even in a thin T-shirt after surgery,
the reduction of chest protrusion
is clearly visible.

Before gynecomastia surgery,
the chest volume and projected nipples
were noticeable through clothing,
and the asymmetry significantly
affected the overall fit.

After correction, the uneven chest
was balanced, and the volume
around the nipples
was refined,
creating a neater, more stable shape.

Now, with just a single thin T-shirt,
a smooth and natural chest line
is visible.

This patient had a clear asymmetry,
diagnosed at different Simon grades
between left and right.
The difference was obvious
even in the external appearance.

During surgery, not only the enlarged
gland tissue
but also the residual fat
in the lower and lateral chest were removed,
resulting in a smooth and balanced contour.

The pronounced size difference
between both sides was naturally corrected,
and now the chest size and projection
remain stable.

As long as the patient continues
to care for the surgical area without strain
and maintains body balance in daily life,
the natural and refined outcome
will remain stable long-term.
