In summer, when even a thin short-sleeved shirt
reveals the full shape of the body, many men
with upper body fat or noticeable chest protrusion
find the season uncomfortable rather than enjoyable.

Despite efforts through exercise and diet control,
the chest volume often remains unchanged.
If only the area under or around the nipple
appears prominently bulged, it may indicate
a possibility of gynecomastia.

In this post, we will review a case
where chest protrusion unresponsive to workouts
or weight loss was surgically corrected.
Through this, we examine the diagnosis
and treatment results of true gynecomastia.

The patient, in his early 30s,
had noticed bilateral chest protrusion
since his teenage years.

At first, it was only a minor concern
regarding his appearance, but as time passed,
the persistent chest size that did not improve
began to cause significant stress
about his overall body shape.

Feeling there were limits to improvement
through self-management alone, he decided
to visit Lesarts Plastic Surgery
for an accurate diagnosis
and surgical treatment of gynecomastia.

Before the patient’s gynecomastia surgery,
the chest showed protruding volume beneath both nipples,
with the areola appearing enlarged and swollen.

Palpation revealed a firm nodule under the nipple,
which was the area that caused the patient
the greatest discomfort in daily life.

The nipple itself appeared slightly inverted,
and mild ptosis (sagging) was observed
in the lower chest region.

A detailed ultrasound examination confirmed
clear glandular overgrowth in both breasts.
Based on these findings, the patient was diagnosed
with true gynecomastia, Simon Grade 2B.

Accordingly, the following surgical plan
was explained to the patient:

1.Gland Excision
An incision around the areola was made
to remove the abnormally enlarged glandular tissue,
reducing the overall chest size.

2.Liposuction
Fat was removed from the lower chest curve
and lateral chest areas, creating a smoother,
more natural and balanced chest contour.

3.Adhesion & Inversion Prevention
After gland removal, deep tissue was repositioned
to prevent nipple inversion or adhesion,
ensuring a stable healing process.

At 2 months after gynecomastia surgery,
we reviewed how the patient’s
chest protrusion and chest line
had improved compared to before surgery.

Before male chest reduction surgery,
the volume around both nipples
was clearly protruding,
and the heavy chest line
drew attention from every angle.

From the side and oblique views,
the lower chest beneath the nipple
looked sagging and heavy,
giving a compressed chest shape.

At 2 months post-op,
removal of glandular tissue and fat
dramatically reduced the chest volume,
making the chest appear flatter.

Additional liposuction on lower chest
and lateral chest areas
created a smoother contour,
refining the overall chest balance.

Most notably, the reduced nipple volume
brought symmetry and balance,
resulting in a natural male chest line.

Even under a thin cotton T-shirt,
the post-surgery change is clearly visible.

The previously protruding chest shape
seen through the T-shirt
is now smoother and stable,
showing a refined male chest contour.

Because of his larger upper body,
the chest protrusion appeared
more prominent and unbalanced,
but surgery corrected this issue,
improving the T-shirt fit as well.

These results were possible because
not only the gland tissue
but also lower and lateral chest fat
were addressed through a
comprehensive gynecomastia surgery plan.

For long-term maintenance,
the patient was advised to continue
exercise, diet control, and weight care
to preserve results and build
a more balanced upper body line.
