Men experiencing chest protrusion
face more than just an appearance issue.

Wearing a thin T-shirt, exposing the upper body
at the pool or sauna, or simply changing clothes
in front of a mirror often leads to self-consciousness
and a tendency to shrink away.

Gynecomastia in men is especially stressful
because it is difficult to resolve on its own.

Even with workouts and weight loss,
the persistent chest protrusion does not reduce,
leaving men uncertain about what else can be done.

This condition is more than a body shape concern.
It affects confidence and impacts daily life,
making it a significant issue.

Today, we will review the case of a male patient
who had struggled with gynecomastia since adolescence,
and examine his surgical before-and-after results
along with the recovery process.

This male patient in his late 20s
had noticed chest protrusion
on both sides since adolescence.

At first, he thought it was a
temporary change from growth,
so he wasn’t too worried.
However, even into adulthood,
the protruding chest and
enlarged areola persisted,
causing increasing discomfort
in his daily life.

The most stressful moments were
when wearing a thin T-shirt,
in sportswear, or situations
requiring upper body exposure.
He often lost confidence,
feeling self-conscious
about his appearance.

He tried exercise and
diet control to reduce weight,
but the chest bulge did not improve,
leaving him frustrated.

In particular, the swollen nipples
and areolar protrusion became
more pronounced,
causing greater psychological stress
both during workouts
and in daily life.

At first, he assumed it was simply
a body shape issue from weight gain,
but as the discomfort persisted,
he suspected there might be
another underlying cause.

He decided to seek a precise diagnosis
and treatment consultation,
which led him to Lesarts Plastic Surgery.

Before a detailed consultation,
we visually examined the patient’s condition.
Both sides of the chest showed clear protrusion,
and the areola and nipple appeared slightly swollen.

Although overall skin elasticity was good,
a firm mass could be palpated under the chest,
raising suspicion of true gynecomastia.

To confirm, we performed an ultrasound examination.
It revealed glandular tissue overgrowth
on both sides, diagnosed as Simon Grade 2A,
a clear case of true gynecomastia
requiring surgical correction.

Based on this diagnosis,
we explained the following gynecomastia surgery plan:

1.Gland Excision
Remove the enlarged glandular tissue precisely,
resolving the root cause of chest protrusion
and areolar enlargement.

2.Liposuction
Refine the contour by removing fat
from the lower and lateral chest,
achieving a smoother and more natural chest line.

3.Adhesion & Depression Prevention
Prevent postoperative adhesion or nipple depression
by evenly filling the excised space
and carefully repositioning the tissues.

Next, we will review the patient’s progress
at 2 months after gynecomastia surgery,
by comparing before-and-after photos
to observe the transformation of the chest contour.

When reviewing the preoperative photos,
both sides of the chest showed enlarged glandular tissue,
protruding visibly even below the areola,
creating a clear bulging appearance.

From the side view, the lower chest volume
extended downward, making the entire chest
look as though it was pushed forward.

At 2 months after gynecomastia surgery,
the removal of glandular tissue combined with
liposuction of the lower and lateral chest
produced dramatic improvement.

The previously swollen lower chest volume
was significantly reduced, and the chest contour
was reshaped into a flat, masculine line.

In addition, the once projected chest profile
was corrected into a smoother, natural flow,
settling into a more stable and athletic contour.

When comparing the before-and-after photos
wearing a white T-shirt,
the effect of gynecomastia surgery
is even more pronounced.

Before surgery, the chest projected forward,
creating a bulky look that constantly
drew attention and reduced confidence,
even in everyday clothing.

At 2 months post-op,
the removal of glandular tissue and
lateral chest liposuction clearly reduced chest size,
refining the contour into a smooth and natural line.

Unlike before, the patient can now appear
more comfortable and confident,
even in situations requiring upper-body exposure.

However, it’s important to remember that
gynecomastia cannot be corrected
through exercise or weight loss alone.

In cases of true gynecomastia caused by
glandular overgrowth, accurate diagnosis
and surgical treatment are essential
for fundamental improvement.

As explained, for patients with
