Many men who come in for surgery consultations due to gynecomastia often mention that they have tried dieting and chest exercises extensively.

However, gynecomastia is a condition where the breast tissue enlarges abnormally in men due to the overgrowth of glandular tissue, not just fat tissue. Therefore, it should be viewed differently from the breakdown of fat tissue that occurs through exercise or diet.

In cases diagnosed with gynecomastia due to glandular growth, surgical treatment becomes necessary.

The primary goal of gynecomastia surgery is to remove the excess glandular tissue, restore the shape of the chest, and potentially correct poor posture such as rounded shoulders or a hunched back.

Therefore, if you are experiencing discomfort or stress due to gynecomastia, it is important to consult with a medical professional to explore treatment options.

This patient is in his mid-20s and has been experiencing gynecomastia since his teenage years. He noticed both of his breasts becoming protruded like a woman’s, which led to the development of gynecomastia symptoms.

Over time, he continued to experience discomfort in his daily life. Even after becoming an adult and serving in the military, the discomfort due to the size of his chest persisted.

Recently, as he began his social life, he gained weight, which further increased the volume in his chest area and added more stress. Additionally, he noticed lumps around his areola, with a slightly raised shape, which caused further discomfort.

Seeking relief from the enlarged chest and associated discomfort, he visited our clinic for consultation.

During the gynecomastia consultation, it was visibly evident that both sides of the chest had significant protrusion. As the patient mentioned, there was also some slight puffiness around the areola, making it appear enlarged.

The diagnosis revealed that the patient fell under Simon Grade 2A, and an ultrasound confirmed the need for glandular tissue excision on both sides, indicating true gynecomastia.

Additionally, there was a considerable amount of accumulated fat in the chest, which led to the diagnosis of complex gynecomastia.

As a result, the patient required gland excision surgery to remove the overgrown glandular tissue, addressing the core volume issue in the chest. To prevent any indentations or imbalances from occurring due to the removal of the tissue, a tissue rearrangement procedure was necessary to reshape the chest.

Given the patient’s slightly reduced skin elasticity, we advised careful postoperative monitoring to assess potential skin sagging and other post-surgical concerns.

When comparing the photos taken before the surgery and 3 months after, it is clear that the protruding chest volume has significantly reduced, and the chest shape has returned to a more normal appearance.

Additionally, the patient previously had slightly protruding nipples and a wider areola, which made the chest appear larger than average. However, through gynecomastia surgery, the glandular tissue was excised, correcting the chest size, and the shape of both the nipples and areolas was also improved.

In the before-and-after photos taken while wearing a thin T-shirt, you can clearly see the improvement in the chest volume.

The protruding nipple area and chest volume that were present before the surgery have disappeared, resulting in a flatter, more masculine chest shape.

The patient was very satisfied with the surgery, and we also considered this case to be one with great post-surgical satisfaction.

This patient had slightly lower skin elasticity than average, so we advised him to be more attentive to wearing compression garments for stable recovery.

By the 3 month mark, when the patient entered a more stable phase, we also emphasized the importance of maintaining consistent chest exercises, diet, and overall lifestyle management for even better results from the gynecomastia surgery.
